Doctor's approval required
Based on your medical statement, a physician needs to review it and confirm you're fit to dive. There are two ways to do this:

They review your questionnaire and sign off online — no account needed.

Annual activity release
Covers recreational diving, freediving, snorkeling, boat travel, and related Sea Hunt activities for 12 months. A revised agreement requires a new signature.
Waiver details
Your signed copy includes English, Spanish, signatures, choices, and an evidence certificate.

A parent or legal guardian must open your profile from their Sea Hunt family account so you can both sign.
